At FMC, we use electronic medical records.

Upon arrival, you can expect to sign all legal documents electronically along with having your picture taken for your electronic file. All medical documents are maintained with the utmost confidentially and are never released without the patient's written consent. There is a minimal fee involved for all records requested, except when faxed to a collaborating physician.

No Food

We strive to maintain a clean environment at our clinic. Therefore, we ask that no food is brought into the waiting area or the exam rooms.

No Cell Phones

We also believe in providing our patients with our undivided attention when in the exam room. Therefore, we ask that cell phones are not used during exams.


At FMC, we prefer for our patients to call and schedule their appointment. During the initial telephone contact, you will be asked for your insurance information. Please have all insurance information available.

What To Bring

Your Insurance Card

A Valid ID Card

We ask that you bring your insurance card and a valid identification card upon arrival at each visit. Your insurance will be verified before each visit.

We will take walk-in appointments.

Please be advised, walk-ins may have a longer wait time than scheduled appointments. Scheduled appointments are taken as close to their appointment time as possible. If you prefer to walk-in, we will need to verify your insurance before you are seen by the provider and you must have a valid identification card available as well. Walk-ins are seen by the provider on a first-come-first-serve basis at a time that does not conflict with a scheduled appointment.
